Create a Workflow Form
Some Workflow Actions will require a Form to request or override values during an Entitlement request. In most cases, it will be necessary to configure the form before any actions can refer to them.

Give the form a name and optional description, then click Add Form Item to further define the form fields.
Select the desired form item from the subsequent list and provide details as requested. The form items are as follows:
Note
The Visibility Per Step tab allows you to define the workflow steps on which this form item will be visible, editable, and/or required.
Checkbox
The checkbox feature allows administrators to add a binary choice for user input.

Display Name | Enter a name to display above the form field |
ID | Enter a unique identifier for this form field. This identifier must be unique within the workflow |
Description | Enter an optional description for this form field |
Hide Values from Recipient | Enable this to disable the field value from being visible to the user within the form (used to obscure sensitive information) |
Date Picker
The Date Picker provides a field where a user can choose a date.

Dropdown
The Dropdown field provides a list of options that the user can select from. Administrators can define the viewable options and associate them with system values within the workflow.

File Attachment
The File Attachment option allows users to upload a required file to complete or respond to the entitlement. Files are uploaded into the server's clustered file system, and anyone who can see the form values for a particular entitlement would have access to the associated files.

The Input Field element provides users with a text field into which they can enter required data.
